How to register

We have an open list and welcome requests for registration from patients living in or moving to the practice area.

Our reception staff will be happy to guide you through the procedure and eligibility can be quickly confirmed from your address so please provide proof by way of a recent utility bill along with one form of photo ID.

You can register by either:

If you have completed the form online, when you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.  Please be aware this will not automatically mean you are a patient at the Surgery.  We still ask you to come to the surgery bringing us proof your your identity and address and to sign the form.

We will keep the form in reception but it is your responsibility to attend the surgery to complete the registration process.  Be aware we do not cover all of the Stockton area and the areas we do cover are listed on the previous page.  If you are in doubt please contact us and we will inform you if we cover your home.

You will have a named, accountable doctor who is responsible for coordinating your care. You can still talk to or make appointments to see any of our doctors or nurses, not just your named GP.

All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.

Medical treatment is available from the date of registration. Please contact reception for further information.

Temporary patients

If you are not registered with a GP you will be unable to see one unless you have a serious emergency. In most other cases, calling 111 is a better option.

If you are not registered with a GP but need treatment at a GP Surgery you will need to complete a temporary registration form.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
